Responsibilities of Social Worker

• Provide psychosocial support to residents and their families

• Conduct social service assessments and maintain accurate documentation

• Assist with care planning and participate in interdisciplinary team meetings

• Coordinate discharge planning and transitions of care

• Facilitate resident and family meetings as needed

• Assist residents in accessing community resources and support services

• Address concerns related to resident rights, adjustment, and quality of life

• Support admissions and orientation processes when necessary

• Ensure compliance with federal, state, and facility regulations

Qualifications for Social Worker

• Bachelor's Degree in Social Work required

• Social work experience in a skilled nursing facility, rehabilitation center, healthcare setting, or long-term care environment preferred

• Knowledge of discharge planning, care coordination, and resident rights

• Strong communication, organizational, and problem-solving skills

• Ability to work collaboratively with residents, families, physicians, and interdisciplinary team members

• Experience with electronic medical records preferred
