Responsibilities
- Provide direct patient care for long-term care residents under supervision of a registered nurse
- Administer medications and document administrations per plan of care
- Monitor vital signs and report changes promptly
- Assist residents with activities of daily living and maintain comfort and safety
- Maintain accurate charting and documentation in the electronic health record
- Collaborate with the interdisciplinary team and follow individualized care plans

Requirements
- Active LPN license in good standing
- Availability to work evenings and weekends as required by the shift
- Strong written and verbal communication skills
- Ability to perform essential nursing duties in a long-term care setting
- Experience in a skilled nursing or long-term care environment is preferred
