Discharge & Care Coordination Specialist

Yale NewHaven Health

Bridgeport Hospital, part of Yale NewHaven Health, seeks a discharge planning coordinator to ensure smooth transitions from hospital to next care setting. You will collaborate with Care Management, external agencies, and patients/families to develop effective discharge plans.
