What you will do:

The Lung Transplant Evaluation Coordinator is responsible for coordinating and facilitating all aspects of the lung transplant evaluation process for potential transplant candidates. This role serves as the primary point of contact for patients caregivers referring providers multidisciplinary team members and external healthcare organizations. The coordinator ensures timely completion of required testing consultations education and regulatory documentation while promoting exceptional patient-centered care and compliance with transplant program standards.

Key Responsibilities

60% CARE COORDINATION

Become the central hub of communication and organization for pulmonary hypertension patients as you:

• Coordinate care using established clinical protocols.
• Review diagnostic tests labs and clinical data to support decision-making.
• Ensure accurate documentation in progress notes flow sheets the Phoenix database and the EHR.
• Collaborate with a multidisciplinary team including physicians social workers financial coordinators and nursing staff.
• Manage triage calls and partner with physicians to assess and address patient care needs.
• Participate in program and committee meetings.
• Submit clinical information to insurance carriers for precertification/authorization.
• Provide cross-coverage to fellow coordinators as needed.

10% PATIENT & STAFF EDUCATION

Help patients and families better understand their condition by:

• Creating and maintaining educational resources for patients families staff nurses and program teams.

10% DATA MANAGEMENT

Support program excellence by:

• Collecting reporting and submitting required data to the Pulmonary Hypertension Association in alignment with accreditation standards.

10% QUALITY & ADMINISTRATIVE SUPPORT

Elevate program performance by:

• Preparing quality metrics attending protocol meetings and supporting administrative initiatives.
• Participating in Quality Assurance/Performance Improvement activities to strengthen outcomes and operations.

OUTREACH & EXPERT CONTRIBUTION

Expand the programs reach and reputation by:

• Participating in community outreach efforts and traveling to partner facilities as needed.
• Serving as a pulmonary hypertension content expert at local regional and national levels.

What you will need:

• 3 years of professional nurse experience; at least 1 year of previous transplant outpatient or dialysis experience highly desirable.
• ADN required; BSN bachelors degree in nursing preferred
• Current IL RN license
• CPR (Cardiopulmonary Resuscitation)
• Preferred: Specialty certification in clinical or functional area of nursing granted by a national nursing organization
• Preferred: National Certification as Transplant Coordinator or other related specialty preferred.

Compensation:

Salary Range: $39.48 - 65.14 per hour

Actual compensation will fall within the range but may vary based on factors such as experience qualifications education location licensure certification requirements and comparisons to colleagues in similar roles.
