Job Category: Direct Service Requisition Number: CASEM006271 Posted: April 7, 2026 | Part‑Time | On‑site Locations Showing 1 location The Case Manager provides outreach and support to individuals who are engaged in the re‑entry process as well as other justice‑involved members of the community. They have the clinical and treatment supports necessary for success, including medication‑assisted treatment, therapy, and other community services. The position requires community support and case‑management work in the jail, the outpatient clinic, and the general community. The Case Manager assists clients in attaining their clinical treatment‑plan goals in outpatient and other levels of care and works to mitigate barriers to success. Responsibilities Provide service coordination and linkages to the community. Deliver outreach services to clients, providing mobile services in a number of community or institutional settings including inpatient units, homes, day programs, etc. Assist clients in obtaining benefits, housing, and health care as needed. Provide crisis support and participate in the development, revision, and utilization of crisis prevention and/or safety plans. Encourage and facilitate utilization of clients’ natural support systems as well as recovery‑oriented, peer support, and self‑help supports and services. Assist in data collection and tracking for grant evaluation. Requirements Ability to pass certification for CPR and First Aid. Valid driver’s license and reliable transportation for business use. Ability to counsel and support clients with psychiatric and/or substance use disorder diagnoses. Demonstrated knowledge of trauma‑informed care. Lived experience relative to psychiatric and/or substance use disorder preferred. Recovery Coaches welcome. Bilingual encouraged to apply: Spanish/English.
