We are seeking dedicated Pharmacists to lead and support pediatric patients’ medication management needs. As a pharmacist you will play a pivotal role in our inpatient pharmacy environment, ensuring the safe and effective use of medications for children. Position Duties Review and manage medication orders for therapeutic appropriateness based on age, weight, height, laboratory values, diagnosis, and other patient and clinical information. Verify orders accurately and timely. Screen for and manage clinical alerts/conditions such as allergies, interactions, therapeutic duplications, and adverse drug reactions. Problem‑solve medication order issues and recommend appropriate alternative therapies to prescribers. Comply with Pharmacy and Therapeutics Committee directives related to formulary management. Document clinical interventions to meet cost‑containment and evidence‑based goals. Prepare and dispense sterile and non‑sterile medications in compliance with department policies, federal, state, and local laws. Demonstrate clinical pharmacy skills in assessing and formulating medication therapy recommendations that are safe, evidence‑based, patient‑appropriate, and cost‑effective. Participate in decentralized pharmacist assigned duties when applicable. Assist in developing guidelines, policies, and monitoring strategies for medication use. Actively participate in ongoing education and training of peers, residents, interns, technicians, and learners; orient new employees. Directly supervise technical, intern, and support staff to ensure accuracy in medication preparation and dispensing; coordinate daily workflow. Serve as liaison between pharmacy and medical/nursing staff for medication management issues; participate in hospital and department operational and quality improvement initiatives. Communicate problems and patient needs to incoming staff to enhance continuity of care; respond promptly to all communication modes. Initiate, process, and complete incident and adverse drug reaction reporting and documentation; engage in drug‑use evaluation, quality assurance, and performance‑improvement activities. Maintain investigational drug study records as required by sponsors and accrediting bodies. Provide evidence‑based medication education and information to patients, caregivers, and staff; deliver counseling for outpatient prescriptions. Perform miscellaneous duties as requested. Position Qualifications Education: Bachelor’s Degree (BS) or Doctorate Degree (Pharm.D.) in Pharmacy or graduation from an accredited pharmacy school within the next 6 months. (Required) Completion of a post‑graduate year 1 (PGY‑1) pharmacy residency program accredited by the ASHP. (Preferred) Experience: Experience with information systems and automated technical equipment. (Required) One year of experience in a hospital pharmacy practice or equivalent. (Preferred) One year of previous pediatric patient population experience or equivalent. (Preferred) Special Skills: Experience with various computer software, including MS Office Suite or equivalent. (Required) Ability to problem‑solve and collaborate with team members to support a cohesive work environment, demonstrating empathy, teamwork, and willingness to give and receive constructive feedback. (Required) Additional Requirements: Current licensure as a Pharmacist or eligibility by reciprocity or examination by the Arizona State Board of Pharmacy. (Required) Current BLS and PALS or ACLS for Healthcare Provider certification from the American Heart Association.
