The Home Care Coordinator Discharge Planner specializes in coordinating home care services and resources for patients transitioning from hospital to home, acting as a discharge planning resource without providing hands-on clinical care. Work Location: Anne Adrundel , Annapolis Maryland Weekly Working Hours: Days, PRN Job Requirements: Bachelor of Science in Nursing (BSN) required Licensure in clinical discipline (RN) Active CPR Two (2) years experience in clinical nursing required , previous home care or home care discharge planning experience preferred Knowledge of Medicare Conditions of Participation preferred Thorough knowledge of home care community resources, agency policy and procedures, federal and state regulations and third party regulations for reimbursement of home care services preferred. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
