Title
Inpatient Case Manager - Social Work

Schedule
Typical schedule: Sunday-Thursday (subject to change based on staffing)
Weekend requirement: Every other weekend
Holiday commitment: Must work 3 of the 5 listed holidays (Thanksgiving, Christmas Eve, Christmas Day, NYE, NYD)

Assignment Details
Caseload: 20-23 patients
Orientation: 40 hours

Requirements
- Required: 1 year of acute care hospital experience
- Required certifications: BLS; MSW or LCSW
- Experience with EMR: EPIC

Roles & Responsibilities
- Conduct initial assessments and coordinate discharge planning for adult and geriatric patients.
- Develop and revise plans of care in conjunction with the interdisciplinary team.
- Liaise with insurance case managers and utilize management for prior authorizations.
- Manage referrals, bed assignments, and DME coordination.
- Document care activities per plan and provide crisis intervention as needed.

Compliance / Notes
- Must commit to working in various facilities within the state.
- RTO limit: 7 days (exceeding may delay offer approval).
