The listed pay range or pay rate reflects compensation for a full-time equivalent (1.0 FTE) position. Actual compensation may differ depending on assigned hours and position status (e.g., part-time). Pay Range $16.75 - $20.55 Scheduled Weekly Hours 24 Position Overview SUMMARY Basic Purpose of the Job) The Certified Medication Assistant assists Residents with Medication Administration & treatments under the direction of the RN. The CMA shall also be responsible for providing personal care to Assisted Living Program participants residing in publicly-subsidized housing complexes with agreements with CH ALP. The CMA is delegated the responsibility of administering medications and wound care under the supervision of the RN. The CMA also aids with ADLs, meal preparation, light housekeeping and maintaining the cleanliness and neatness of resident apartments, and residents’ laundry. The CMA may also schedule transportation and medical appointments for residents as needed. Minimum Requirements High School Diploma or GED Certified Nursing Assistant Certificate or Certificate Home Health Aide Certificate 1 year of CNA/CHHA experience Successful completion of medication administration training course & statewide standardized, written competency test. NJ Medication Aide certification. AHA Heartsaver CPR AED. Non-licensed must obtain before end obtain before end of orientation period. Excellent customer service skills. Possess a friendly and caring demeanor. Has excellent bedside manner. Training in multiple age populations. Proficient in Microsoft Word. Good critical thinking skills. Possess ability to work under stressful conditions. Usual Work Day 8 hours may require travel between assisted living program locations. Working in the community with residents who reside in publicly subsidized housing. Reporting Relationships Acts as shift lead ensuring CHHAs, CNAs document in communication book properly, that they are reading communications and following through on communications as needed. Supervise CHHAs, and CNAs to ensure implementation of resident care plan/service plan. Essential Functions Maintains a working relationship with the LPN, RN, DON, Assisted Living Administrator and all other nursing staff, Volunteers, Capital Health LIFE/ Capital Health Administrative Staff, Hospital Employees, Department Heads/Supervisors, Outside Vendors/Contractors, Caregivers, Family Members, and Property/Building Staff. Maintains a professional relationship with all residents. Maintains continuing education requirements for medication administration. Maintains confidentiality of verbal and written information pertaining to residents, ALP operations and other personnel. Maintains residents' privacy and dignity. Encourages residents to their independence. Has a full understanding of fire, emergency and safety procedures. Adheres to policies, procedures, and standards regarding infection control. Has a full understanding of the hazards and safety of oxygen being used by residents. Daily Check phone messages & return calls as needed. Review any new faxes, fax new orders to MDs, Pharmacy, etc. Read communication book & daily logbooks and address any concerns. Check list of residents scheduled for MD/other appointments and ensure progress notes are prepared by nurse to accompany resident, if necessary. Review Narcotic count. Make sure CHHAs, CNAs document in communication book properly, that they are reading communications and following through on communications as needed. Supervise CHHAs, and CNAs to ensure implementation of resident care plan/service plan. Assist residents with medication administration following proper medication techniques as learned in CMA course. Be able to properly demonstrate the FIVE RIGHTS of medication administration and Safe Medication Practices. Maintain frequent close communication with the RN. Properly identify medication and residents before administering medication. Document on MARs when medication has or has not been given. Notify LPN/RN if resident not given or refuses medication. Notify LPN/RN immediately if medication error has occurred and document appropriately. Check all meds daily and call pharmacy for needed refills. Administer medications including pre-filled insulin pens as per MD orders Monitor and document Glucose levels Monitor and provide wound care as delegated by RN Quarterly, direct observation and supervision of the medication administration task by the RN. Receive medications from pharmacy and document that they were delivered. Provides AM & PM personal care to assigned residents including partial or complete bath, oral hygiene, hair care, incontinence care including changing adult briefs as needed. Provides ostomy care. Takes weights and vital signs (TPR & BP) monthly & as requested by nurse, record and report results to nurse. Provide ambulation assistance as needed. Assist with transfer to & from bed. Report any change in resident status to nurse immediately. Respond to emergency calls & assist nurse as needed. Report any accidents, injuries, or complaints to nurse or administration immediately. Deliver supplies and equipment to residents. Provide light housekeeping. Cleans refrigerator, stove/oven, cabinets and silverware drawers as needed. Vacuum and dust as needed. Assist with laundry: washing, drying and folding laundry and putting it away. Strip and remake bed as needed but at least 1x/week. Clean the bathroom including bathroom floor, vanity, toilet, and shower/tub (with tub wand provided by resident). Removes trash from resident apartments daily. Assist resident with meal service as needed. Ex: meal preparation & cutting up foods, set-up, clearing and washing dishes. Report any unsafe equipment to nurse/administrator. Maintain the cleanliness and neatness of the wellness office, laundry room, equipment, and inventory supplies. Removal of trash from wellness offices and (Trent Center East) laundry rooms. Abides by established guidelines of the facility and governing regulations. Any other responsibilities outlined in the Certified Home Health Aide job description or deemed necessary by the administrator. PHYSICAL DEMANDS AND WORK ENVIRONMENT Frequent physical demands include: Sitting, Standing, Walking, Climbing (e.g., stairs or ladders), Carry objects, Push/Pull, Twisting, Bending, Reaching forward, Reaching overhead, Squat/kneel/crawl, Wrist position deviation, Pinching/fine motor activities, Keyboard use/repetitive motion, Taste or Smell, Talk or Hear. Work at assigned housing provider partner location with travel to other locations where the Program has agreements to provide service. Occasional physical demands include: Climbing stairs in buildings to provide care/service to Program participants/residents. Continuous Physical Demands Include Lifting Floor to Waist 35 lbs. Lifting Waist Level and Above 35 lbs.
