RN: Case Manager

Job Description Join a dedicated team of Case Management Registered Nurses in an acute care hospital setting where you’ll coordinate patient care and facilitate smooth discharges. This 13‑week contract offers a Monday through Friday schedule with 8‑hour day shifts from 08:00 to 16:30. You will be responsible for managing a caseload with a ratio of 1:20, leveraging your two years of acute hospital experience to ensure optimal patient outcomes. The role requires effective communication with hospital staff and patients, and clinical acumen to navigate complex care plans. Be prepared for a setting that values professionalism and efficiency—almost like being in charge of your own care "department" within the hospital.
