Case Manager

Dallasact seeks a dedicated case manager to deliver services under general supervision for county programs. You will identify needs, assess eligibility, and provide counseling, outreach and referrals to clients. Responsibilities include interviewing clients, verifying documentation, preparing case plans and coordinating with other departments and community groups to ensure effective service delivery.
