Licensed Specialist Clinical Social Worker

Description

  • Provide trauma-informed, evidence-based mental health therapy to individuals, families, and groups in an outpatient setting. You'll collaborate with treatment teams, develop person-centered plans, and deliver services that support client progress and overall well-being.
  • Provide mental health therapy services to a wide variety of client systems in an outpatient setting, diagnosing and treating mental health disorders.
  • Deliver trauma-informed, evidence-based therapy services to individuals, families, and groups.
  • Collaborate with other treatment team members, natural supports, and external providers to ensure a wrap-around approach is provided for continuity of client care.
  • Create and maintain person-centered treatment plans based on clients' unique needs.
  • Monitor client progress through ongoing assessment and timely documentation according to current agency standards.
  • Demonstrate sensitivity to all cultural and ethnic differences.
  • Complete agency trainings up to and including certification in evidenced-based treatment models.
  • Provide psycho-educational services to the community as assigned by Supervisor or Director.
  • Conduct mental health assessments and develop individualized treatment plans; provide these plans to the patients' primary care provider and care team.
  • Lead individualized or group therapy sessions.
  • Run preventative mental wellness classes or workshops and make treatment referrals.
  • Maintain confidentiality of records relating to clients' treatment.
  • Interview patients to gather information about their backgrounds, needs or progress.
  • Assess patients for risk of suicide attempts.
  • Fill out and maintain client-related paperwork, including federal- and state-mandated forms, client diagnostic records, and progress notes.
  • Perform crisis interventions with patients.
  • Act as a patient advocate to coordinate required services or to resolve emergency problems in a crisis situation.
  • Modify treatment activities or approaches as needed to comply with changes in client's status.
  • Evaluate the effectiveness of counseling programs on clients' progress in resolving identified problems. And moving towards defined objectives.
  • Encourages patients to express their feelings and discuss what is happening in their lives helping them to develop insight into themselves or their relationships.
  • Prepares for, attends and participates in team meetings, quality improvement projects, team huddles and scheduling huddles with provider and registration.
  • Establish, promote, and maintain positive relations with community agencies/services.
  • Supports and abides by the mission, values, and policies of CHCS in all activities.
  • On-going compliance with CHCS educational requirements.
  • All other duties as required.

Requirements

Master's degree in psychology or related field

Licensed specialist clinical social worker (LSCSW)

Licensed Masters Social Worker (LMSW)
