Granite Peak Home Health is seeking a licensed Physical Therapist to join our home health team. You will provide physician-prescribed PT services under the plan of care, work with PTs and other clinicians, and help return patients to their prior level of function. Responsibilities include patient assessments, documentation, patient/family instruction, and collaboration on care plans.
