RN Clinical Supervisor- Per Diem

Apply Today Become a valuable member of Modivcare PCS. Position Overview All Metro Health Care , a Modivcare Personal Care Service, is looking for a Per Diem RN Supervisor- Home Care. You will have the opportunity to make a meaningful impact on the lives of our clients and foster the professional growth of our home health aides. Pay & Location Pay per Visit Sup/Post Hospital: $60 Reassessment: $90 Initial Admission: $120 weekday / $180 weekend Visiting Client Territory: Mount Vernon Schedule: Visits planned with the required presence of the patient's aide. Office Address: 6 Gramatan Ave. #300 Mt. Vernon, NY 10550 Mileage Reimbursement: Provided at the current federal rate Responsibilities Performs on-site supervisory and teaching visits to evaluate caregivers. Provides clinical orientation to caregivers on assigned home care cases. Participates in the training of new nurses and in‑services for field staff. Performs initial nursing assessments. Counsels patient/caregiver in meeting nursing and related needs. Assigns and coordinates services provided. Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient’s condition/needs. Prepares clinical progress notes ensuring any changes in the patient’s condition is reported. Monitors clinical records for complete and accurate documentation. Qualifications Registered Nurse (RN) CPR Certified 1 year of Home Care experience required 1 year of supervisory or management experience required Valid NYS Driver’s License, insurance, registration, and reliable car Physical Demands The physical demands described are representative of those that must be met by an employee to successfully perform the essential functions of the position.
