Description

• Conduct thorough evaluations for patients in both inpatient and outpatient environments and craft customized treatment plans based on individual needs
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functional capacity
• Collaborate with a multidisciplinary team including nurses, physicians, case managers, and other rehabilitation experts to ensure optimal patient outcomes
• Educate patients and their families on therapeutic targets, discharge plans, and home exercise regimens
• Maintain accurate records of evaluations, treatments, and patient progress in line with facility and regulatory standards
• Engage in care team rounds and interdisciplinary meetings to foster coordinated care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification is preferred
• Specialized certifications (e.g., in neurology or geriatrics) are considered an asset but are not mandatory

Skills

• Previous experience is highly desirable
• Capacity to manage patients facing complex medical and mobility challenges
• Excellent communication skills and an ability to work collaboratively
• Familiarity with digital charting and clinical documentation systems
