Responsibilities
- Perform initial physical therapy evaluations and periodic re-evaluations to assess function, pain, and goals
- Develop and implement individualized treatment plans to improve mobility, strength, balance, and daily activity participation
- Deliver evidence-based therapies including therapeutic exercise, gait and balance training, ROM, and functional training
- Document patient progress, plan of care, and any changes in status in the appropriate medical records
- Collaborate with an interdisciplinary care team to coordinate care, progress toward discharge, and ensure smooth transitions of care
- Maintain patient safety, adhere to infection control and HIPAA requirements, and comply with applicable regulations

Requirements
- Active New York State Physical Therapist license
- BLS certification
- Prior experience in a skilled nursing care setting (required)
- Ability to work in a long-term care environment and adapt to varying patient needs
- Availability for Saturdays on a PRN basis
