About the Role:

As a Home Caregiver, you will be responsible for delivering high-quality, personalized care to our patients, including:

• Assisting with daily living activities such as bathing, dressing, and grooming
• Managing medication schedules and administering medications as needed
• Providing emotional support and companionship to patients and their families
• Maintaining accurate records and communicating with patients' families and healthcare teams
• Promoting independence and dignity for all patients

Responsibilities:

• Provide compassionate and individualized care to patients in their homes
• Assist with light housekeeping, laundry, and meal preparation as needed
• Monitor and report changes in patients' conditions to healthcare teams
• Maintain confidentiality and adhere to HIPAA regulations
• Participate in ongoing training and education to stay up-to-date with best practices and industry developments
• Collaborate with other healthcare professionals to ensure seamless care coordination
