PLACE OF PERFORMANCE : Dental Hygienist Services will be performed in the Health Services Department at FCC Pollock. The physical address is 1000 Airbase Road, Pollock, Louisiana.

SCHEDULE : A session is defined as one (1) hour. The contractor will normally provide services for eight (8) sessions per day. The anticipated schedule for one (1) dental hygienist is Monday through Friday, excluding Federal holidays from 7:30 AM to 4:00 PM, with a 30 minute unpaid meal period. These hours may be adjusted to meet the needs of the institution which will be agreed upon with Contractor and the Government. The contractor shall not normally exceed 40 hours per week.

DESCRIPTION OF DUTIES : All duties shall be performed in accordance with standards and methods generally accepted within the field of Dental Hygienist, and compliance with all Federal and State policy and regulations. The contractor shall adhere to all standards and regulations associated with the Joint Commission (JC) and American Correctional Association Foundation/Core Standards for Adult Correctional Institutions.

The contractor shall provide the following Dental Hygienist services, which includes, but are not limited to, the items listed below. • Provide scaling and root planning (SRP) procedures, periodontal maintenance, and/or prophylaxis as indicated on the adults in custody who are referred by the dental staff. • Perform comprehensive periodontal charting, probing, and documentation. 15BGCS26Q51900001 Page 7 of 42 • Provide thorough individualized oral hygiene instructions for each patient. • Monitor oral hygiene and periodontal condition on each patient seen for dental prophylaxis. • Complete necessary dental x-rays as specified by the dentist. • Sterilize instruments used for prophylaxis and other instruments as specified by the dental staff. • Perform necessary paperwork related to scheduling patients for dental prophylaxis as directed by the dental staff. • Administer local anesthesia as needed.

EDUCATION AND EXPERIENCE : The Contractor is required to be certified/licensed as a Registered Dental Hygienist in any state in the United States or United States Territory and must submit photocopies of their license and/or board certification, three letters of reference from professional peers and sign a consent for release of information for verification of credentials along with their quote. ATTIRE: Additionally, the contractor is required to dress appropriately for the work environment. This includes business attire or scrubs (black and blue scrub uniforms are approved). The attire worn shall also be appropriate for the correctional environment in which the BPA employee will be performing. Jeans (of any color), T-shirts, sandals (of any type), tank tops, and sweatpants/shirts are not considered appropriate professional attire. FCC Pollock will not be responsible for providing a uniform allowance
