This position is primarily remote, with counselors working from home or any quiet space that allows for private patient sessions.   • Collaborate closely with primary care providers to identify and manage mental health and psychosocial concerns, proving to be a vital component of the integrated care team.

  • Implement precise interventions for patients requiring mental health support, ensuring that care is both targeted and effective.

  • Guide primary care providers in making informed decisions on patient referrals, directing them to specialized mental health resources when necessary.

  • Initiate follow-up procedures to monitor patient progress and modify treatment approaches according to individual needs.

  • Develop and implement relapse prevention plans, equipping patients with strategies to maintain stable functioning and prevent recurrence of issues.

  • Identify at-risk patients and design preventive care plans aimed at mitigating the progression of their mental health conditions.

  • Coordinate behavioral health services, ensuring the integration of care extends beyond the primary care setting when appropriate and with patient consent.

  • Document patient progress and diagnostic details meticulously in treatment charts to maintain accurate records for future reference.

  • Maintain continuous communication with primary care providers, sharing updates on patient status and collaborating on comprehensive treatment plans.

Patient Population / Client Focus\n

This role primarily focuses on serving a diverse patient population, including veterans, families, and individuals facing mental health challenges. Our clients often experience a range of emotional and psychological issues, from anxiety and depression to more complex disorders, and it is crucial for the counselor to be adaptable and sensitive to their unique needs.

Qualifications\n

  • Master’s Degree in Clinical Counseling or a related field.
  • \n
  • Current Licensed Professional Counselor (LPC) credential.
  • \n
  • In-depth knowledge of mental health assessment methodologies, treatment planning, and familiarity with developmental psychopathology.

  • Proficiency in family therapy, group therapy, and child psychotherapy is highly desired.

  • Familiarity with the DSM-IV diagnostic criteria and behavioral therapy practices.

Preferred Experience\n

Compensation and Benefits\n

This position offers a competitive salary ranging from USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
