Nurse Practitioner

We are a multi-specialty private physician group seeking a dedicated and motivated Nurse Practitioner to join our growing medical team on a full-time basis . Qualifications: Family Medicine, Internal Medicine, Geriatrics, Rehabilitation, or Post-Acute Care experience preferred Previous experience in skilled nursing, rehabilitation, long-term care, or assisted living is a plus Strong communication, clinical judgment, and organizational skills Willing to train the right candidate Responsibilities: Coordinate patient care with attending physicians, families, nursing staff, and facility teams Communicate regularly with physicians regarding changes in patient status and treatment plans Support the medical team in providing high-quality, patient-centered care Participate in care coordination, documentation, and clinical follow-up Help ensure continuity of care across post-acute and long-term care settings Why Apply?
