Responsibilities

• Assess and treat patients with movement, strength, and postural concerns to improve function.

• Develop personalized treatment plans aligned with patient goals and progress.

• Document treatments, progress notes, and outcomes in the electronic health record.

• Monitor patient outcomes using standardized measures and adjust plans accordingly.

• Ensure compliance with clinical guidelines, safety standards, and regulatory requirements.

• Collaborate with clinicians and therapists to coordinate care within the team.

• Educate patients and caregivers on exercises, home programs, and self-management strategies.

• Maintain high-quality patient care while achieving or surpassing performance targets.

• Demonstrate growth mindset by pursuing continuing education and certification opportunities.

• Participate in pipeline-driven territory planning to optimize patient referrals and access.

• Represent the organization in community outreach and professional networking events.

Experience/Qualifications

• Active Physical Therapy license valid in the applicable state.

• Assess movement, strength, and posture to develop personalized plans.

• Apply standardized outcome measures to monitor progress.

• Ensure compliance with clinical guidelines, safety standards, and regs.

• Meet or exceed performance targets using pipeline-driven referrals.

• Document treatments and outcomes in the electronic health record.

• Collaborate with clinicians to coordinate patient care within the team.

• Educate patients and caregivers on home programs and self-management.

• Demonstrate ongoing professional development and pursue certifications.

• Represent the organization at community outreach and professional events.
