Compensation is based on total years of experience in previous clinical roles.

In this role, you'll care for patients in their homes in the Fort Wayne or Huntington area.

Both RN and LPN are encouraged to apply. 32 hours per week.

You will be working 32 hours each week, Monday through Friday, with Weekend rotations.

POSITION TITLE: RN VISIT NURSE

FLSA STATUS: NON-EXEMPT

REPORTS TO: Clinical Team Leader

POSITION OBJECTIVE:

Performs nursing procedures and care to assigned terminally ill patients by assessing needs and implementing nursing care plans under the coordination of the RN Case Manager.

QUALIFICATIONS:

1. Graduate of an accredited school of nursing.
2. Bachelor’s degree in Nursing preferred.
3. Must have a current RN license to practice in the state of Indiana.
4. Must hold current CPR certification or obtain within the orientation period.
5. Must hold current TB certification or obtain within the orientation period.
6. Infusion skills preferred
7. Must be able to provide those services requiring substantial specialized nursing skill.
8. Prefer one (1) year of recent hospital or home health care experience.
9. Must have a current motor vehicle operator’s license and have access to an automobile with current automobile insurance coverage.
10. Must be able to drive independently to all patient/family clients’ homes.
11. Must be able to safely perform the essential functions of the job without posing a direct threat to the health and/or safety of him/her, or to others in the workplace.
12. Must be comfortable working with hospice patients, with the emphasis on palliative care.
13. Ability to recognize needs and emergencies of hospice and palliative care patients, and provide appropriate interventions.
14. Strong interpersonal skills.
15. Strong assessment skills.

ESSENTIAL FUNCTIONS:

1. Must be capable of functioning independently in the patient's setting within the scope of the Registered Nurse.
2. Implements Agency policies in all work-related situations; orientation of patient/family to hospice benefit services, including identifying the role of team members.
3. Follow state and agency policy and procedures when legally pronouncing a death. Specifics of the death event will be documented in the clinical record; required information is communicated/submitted to appropriate parties. The patient’s body will be handled with respect and dignity and in accordance with individual patient/family requests.
4. Respect the patient's or family's cultural and religious traditions and beliefs.
5. Recognize and maintain professional boundaries in relationships with the patient/family/caregiver.
6. Documents findings accurately, in a timely manner, and on appropriate forms, in accordance with the Agency's procedures.
7. Delivers nursing care to patients, which includes pain/symptom management, teaching, providing direct care, case management, referral, advocacy, and monitoring.
8. Supervises clinically the care delegated to LPNs, Home Health Aides, CNAs, Companions, and/or Homemakers.
9. Instructs LPNs, Home Health Aides, and/or CNAs on specific patient care procedures within approved policies.
10. Maintains a therapeutic relationship with patients/families. This includes the confidentiality of personal matters, patients', coworkers', and the Agency's business.
11. Follows orders from the patient’s physician and informs the physician and other personnel of changes in the patient’s condition and needs.
12. Demonstrates understanding of hospice philosophy and concept of care and considers the patient/family as a unit of care.
13. Assists with the preparation, implementation, and continuing evaluation of the Interdisciplinary Plan of Care.
14. Provides for the emotional and physical comfort and safety of patients. Adheres to Agency policies in all work-related situations.
15. Observes, records, and reports to the appropriate person symptoms, reactions, and changes, including:
    1. General physical and mental condition of patients, and signs and symptoms that may indicate adverse changes.
    2. Stresses in human relationships between patients, between patients and personnel, and between patients and their families and visitors.
16. Orients patient/family to hospice services, including identifying the role of the team member.
17. Anticipates issues related to the patient's impending death and pursues resolution.
18. Remains flexible for patient coverage and accepts responsibility under adverse conditions, such as weather-related crises.
19. Maintains expected and acceptable productivity.
20. Works in a cooperative relationship with other Agency personnel.
21. Seeks appropriate clinical supervision.
22. Participates in the Agency's committees and task forces.
23. Assumes responsibility for continued personal growth and development.
24. Participates in on-call rotation as assigned.
25. Seeks educational opportunities to improve clinical practice and participates in in-service programs.
26. Adheres to all the Agency's policies.

30. Critical Skill Sets and Attitudes:

a. Proficient and timely in all aspects of clinical documentation, including computer data entry.

b. Proficient in blood draws, all routine patient treatments (dressing changes, catheters, etc.), pain and symptom management, care of the patient and family, including pediatric and HIV + patients; Mantoux administration.

c. Proficient in all aspects of IV care, including programming of IV pumps, tubing changes, site care, and subcutaneous access.

d. Display compassion, be a team player, have an optimistic outlook, be adaptive, accountable, and customer-driven.

ESSENTIAL HAZARDOUS PHYSICAL EXPOSURES:

1. Must be willing to travel between patients in inclement weather.
2. Potential threat of physical violence to self. (Security personnel available upon request.)
3. Potential exposure to dangerous animals, such as dogs, rodents, etc.
4. Potential exposure to environmental pollutants.
5. Potential exposure to infectious diseases, needle sticks, blood and body fluids, and hazardous waste.
6. Potential exposure to variable temperatures in the working environment.

CHEMICAL OR HAZARDOUS PHYSICAL EXPOSURES:

1. Potential exposure to substances within the office environment, including copy machine toner, VDT emissions, and cleaning solvents.
2. Potential exposure to chemical/cleaning substances in the patient’s home environment.

ESSENTIAL PERSONAL PROTECTIVE EQUIPMENT REQUIRED:

1. Standard precautions will be consistently used for all patients. Protective clothing and equipment are provided by the employer at no cost to the employee.
2. Seat belts must be worn when traveling during working hours, per state law.

The published compensation range for this position reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. The base pay may be higher or lower based on demonstrated job-related experience.
