Key Responsibilities

• Provide physical therapy services to patients in a home health setting across an assigned service territory
• Meet a productivity expectation of a minimum of 30 points per week
• Complete all clinical documentation by 10:00 AM the following day
• Utilize HCHB (Homecare Homebase) for documentation and care coordination
• Travel throughout the assigned service area to conduct patient visits

Required Qualifications

• Active Physical Therapist (PT) license in the state of Washington
• Minimum of one (1) year of home health experience required
• Proficiency with HCHB (Homecare Homebase) EMR system
• Must reside at least 50 miles from the facility to be eligible
• Reliable transportation and ability to travel throughout the assigned service territory

Preferred Experience / Skills

• Familiarity with the greater Snohomish County area and surrounding communities
• Strong time management and self-directed work habits suited for a home health environment
• Experience managing a high-volume caseload in a home health setting
