New Graduate RN's welcome to apply.* Previous Home Health Care experience preferred.* Minimum 1 year of medical-surgical experience.* Primary Responsibilities: Planning, coordinating, providing and documenting the care of patients in their place of residence.* Clinical practitioner who coordinates and implements patient care specific to Jefferson County.* Ensures that compassionate, engaged care meets or exceeds quality standards.* Care is provided in an efficient and safe manner consistent with the NYS licensure and Samaritan Home Health standard of care.* Utilizes nursing skills to determine the biophysical, psychosocial, environmental, self-care, cultural, ethnic and spiritual and educational needs of the patient/family/significant other to develop, implement and revise the plan of care. In doing so, performs accurate assessments, interprets and correlates finding, establishes plan of care and implements short and long-term goals.* The nurse observes patient progress, records pertinent observations, implements physician and nursing treatment plans and evaluates outcomes.* The RN recognizes their responsibilities as specified by law and abides within those responsibilities.* Provides direct supervision to LPNs and HHAs.* Provides on-call services as required.* BLS required.* ACLS preferred.* Valid NYS Driver's license and proof of auto insurance.* Must be available weekends and Holidays and provide on-call services as required.Samaritan is an Affirmative Action/Equal Opportunity Employer.
