Job Description Supplemental Health Care is seeking a travel Home Health Physical Therapist for a travel job in Saint Petersburg, Florida. Job Details Specialty: Physical Therapist Discipline: Therapy Start Date: ASAP Duration: 13 weeks Hours: 40 hours per week (8‑hour shifts) Employment Type: Travel Qualifications Active, unrestricted Physical Therapist license in the state of assignment Current BLS certification (AHA) 1+ year of recent home health experience (within the last 2 years) Experience conducting PT evaluations and developing therapy plans Familiar with Medicare‑certified home health guidelines and OASIS‑E documentation Experience with EMRs such as Homecare Homebase, NetSmart, or Epic Clearance: background check, driving record, TB and immunizations Reliable transportation with valid auto insurance and ability to travel between patient homes Contract Details $2,120 - $2,336 per week (13‑week contract) Paid by the hour, not by visit 5–6 visits per day (45–90 minutes each, based on complexity) Travel time factored into productivity expectations Key Responsibilities Perform initial and ongoing PT assessments and develop individualized plans of care Provide skilled interventions for strengthening, endurance, pain management, and fall prevention Conduct gait and balance evaluations, home safety assessments, and mobility training Educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies Collaborate with RNs, OTs, SLPs, and physicians to ensure coordinated, interdisciplinary care Document all care in compliance with Medicare, OASIS‑E, and agency standards Benefits Full medical, dental, vision, life, and pet insurance Round‑the‑clock support SHC's Share the Care referral program 401(k) retirement savings program with various investment options Discounted and free online access to CEU courses through Supplemental University Referral bonus Benefits start day 140 1k retirement plan Continuing education provided Discount program Health savings account Equal Opportunity Employment Supplemental Health Care is an Equal Opportunity Employer.
