Therapist - Physical Therapist

Job Summary:

We are seeking a licensed Physical Therapist (PT) to provide in-home physical therapy services to patients. The PT will deliver skilled, evidence-based care to promote patient mobility, safety, and independence within a home setting.

Responsibilities:

• 
  Conduct comprehensive evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits.
  
  
• 
  Develop and implement individualized treatment plans based on patient needs and physician directives.
  
  
• 
  Provide interventions such as gait training, balance exercises, therapeutic activities, and fall prevention strategies.
  
  
• 
  Educate patients and caregivers on treatment protocols, home exercises, and safety precautions.
  
  
• 
  Communicate patient progress, concerns, or changes to the interdisciplinary care team.
  
  
• 
  Oversee the clinical oversight of Physical Therapist Assistants (PTAs), adhering to state laws and agency policies.
  
  

Qualifications:

• 
  Hold a current, unrestricted Colorado Physical Therapist license.
  
  
• 
  Possess graduation from an accredited Physical Therapy program.
  
  
• 
  Maintain a valid drivers license and reliable transportation.
  
  
• 
  Demonstrate ability to practice independently in a home-based environment.
  
  
• 
  Show proficiency or willingness to learn Homecare Homebase (HCHB).
  
  
• 
  Preferred: Home health experience, familiarity with Medicare documentation, and work with geriatric or medically complex populations.
