Home Care RN Manager

St. Paul's PACE Corp, 3444 Camino Del Rio North, SAN DIEGO, California, United States of America. Job Description San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands‑on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. The typical schedule for the position is Monday - Friday, 8:00 AM - 4:30 PM. Under the supervision of a Director of Clinical Services, the In Home Services Supervisor oversees the day‑to‑day operations of the Home Care Services Program for St. Paul’s PACE. The position provides direct supervision to the In Home Services CNA Supervisor who oversees the Certified Nursing Assistants (CNAs), Personal Care Attendants (PCAs), and the In Home Services Assistant of the Home Care Services Department. The position provides supervision visits to home care staff in the field to evaluate skill and performance. It assists the Director of Clinical Services in policy formulation, program development, and implementation. The role is responsible for ensuring compliance with annual budgets and financial objectives, company policies and procedures, and guidelines. The position ensures timely completion of Home Care Services, acts as a liaison between any contracted home health agency and St. Paul’s PACE, and ensures compliance with staff competency requirements. It maintains participants’ medical records through thorough documentation and communicates participants’ changes and progress to appropriate staff. Duties and Responsibilities Oversee the performance and daily operations of in‑home support staff, including caregivers, supervisors, and schedulers, ensuring effective service delivery. Monitor Interdisciplinary Team meetings daily to ensure all concerns related to in‑home services are addressed promptly. Provide orientation and competency assessments and supervise caregivers, CNA supervisors, and schedulers to ensure adequate department staffing and performance; perform supervisory visits to staff in the field. Investigate medication errors that occur in‑home and collaborate with the Clinical Service Manager to develop corrective action plans. Ensure that staff provides supportive services to participants as outlined in the plan of care. Ensure that staff maintains participants’ medical records and fulfills St. Paul’s PACE charting and reporting requirements. Ensure staff provide timely notification to the Clinical Services Manager or designee of any marked change in the participant’s condition. Assist with supply management and ordering; manage home care inventory and distribution across sites. Collaborate with the In‑Home Services CNA supervisor to coordinate personal care activities of all participants in the home. Monitor and complete required data entry in Axis Care for participants receiving In‑Home Services. Maintain overall quality and fiscal accountability for outside vendor services; approve and process vendor invoices in a timely manner. Participate in developing and organizing new staff orientation, training, and ongoing in‑services training for staff and independent contractors. Lead all department meetings and develop agendas in collaboration with In Home services CNA supervisor(s). Collaborate with other nursing leaders in the organization and Center Directors on best practices and in‑home services policies and program development. Support quality improvement initiatives and participate in projects to enhance care delivery and operational efficiency. Educate participants, families, and caregivers regarding in‑home care services; respond to grievances in a timely manner. Follow all St. Paul’s PACE Policies and Procedures and OSHA safety guidelines. Maintain confidentiality of all company procedures, results, and information about participants, clients, or families. Practice universal precautions and follow appropriate infection control procedures. Maintain a safe working environment; follow St. Paul’s PACE Care Safety Policies and Procedures and universal precautions. Participate in continuing education classes and any required staff and training meetings. Perform other duties as required or requested. Qualification and Requirements Education Graduate of a school of professional nursing required. Current California Registered Nurse License or Vocational Nurse License. CPR and First Aid certification required. Experience Minimum of one year’s experience working with the frail or elderly population required. IV Certification preferred. Either training or related experience in the job assigned. Benefits Flexible vacation time – 17 days. Paid sick time. Education reimbursement – up to $3,000 a year.
