Clinical Pharmacist FT

Shift Details Day Shift 3 12-hour shifts per week; days rotate weekly and include 1 weekend per month APPLICATION MUST BE FILLED OUT ENTIRELY TO BE CONSIDERED FOR INTERVIEW PROCESS Scope of Responsibility Antibiotic stewardship and therapeutic drug monitoring Medication consults and drug information support Renal dosing adjustments and clinical interventions Verification and dispensing of medication orders Sterile IV admixture preparation Patient and staff education on safe medication practices Reports directly to the Director of Pharmacy and works closely with nurses, physicians, and other clinical team members.
