A Wraparound Case Manager provides intensive evaluation, consultation and intervention with youth and families, within Heart of Texas Behavioral Health Network catchment area, who are participating in wraparound services and/or YES waiver. Also participates in conducting psychosocial programming with this population as part of a clinical team. EXAMPLE OF WORK PERFORMED
Must have a current valid Texas driver's license and current and continued proof of liability insurance. One year case management preferred. Must be fully licensed or licensure candidate for LPHA.
Education:
A bachelor's or master’s degree in psychology, social work or related field from an accredited college or university. KNOWLEDGE, SKILLS, AND ABILITIES

- Works with youth and their families in mental health, home or office settings.
- Provides case management and some skills training with children and families.
- Performs assessment and psychosocial evaluations and makes treatment recommendations.
- Refers families of consumers to appropriate social welfare and health agencies to obtain assistance.
- Conducts family interview with every family to create a family story.
- Develop a crisis/safety plan with the family and update as needed.
- Help identify other communities and individual supports that can be used by the family to achieve their care plan.
- Creates a Child and Family team for each youth and family.
- Is the primary contact person for each family in the wraparound system on their caseload and will be required to develop a plan of care for that individual family.
- Regular attendance at Family Team meetings.
- Must complete Wraparound training.
- They must comply with all required documentation and data collection to support the program’s implementation and evaluation strategies.
- Other work as assigned.

- Knowledge of emotional problems and treatment approaches.
- Ability to establish and maintain positive working relationships with consumers and colleagues.
- Good work habits and organizational skills.
- Familiarity with computer operation.
- Effective verbal and written communication skills.
- Ability to design and solve problems, ability to develop and deliver case presentations.
- Ability to lift up to 15lbs.
- Excellent organization/filling skills required.
- Ability to travel locally 75% and regionally 25%.
