CareBridge Advance Practice Provider, Nurse Practitioner (Congregate Facilities) Sign on Bonus: $5,000 Location: Arizona, Kansas, Iowa, Indiana, Ohio, and Florida Work Shift: Monday – Friday, 8:00 am to 5:00 pm CST, rotating on‑call This role enables associates to work virtually full‑time, except for required in‑person training sessions, providing maximum flexibility and autonomy. The Advance Practice Provider, Nurse Practitioner (Congregate Facilities) collaborates with company physicians, patients’ other physicians and providers, and family members to develop complex plans of care in accordance with patients’ health status and overall goals and values. Provides clinical and non‑clinical support to patients. How you will make an impact Provides primary and urgent health care via telephone and tele‑video modalities to patients receiving home and community‑based services. Develops and implements clinical plans of care for adult patients with chronic and complex conditions. Conducts history, physical exam, and diagnostics as needed and creates treatment plans aligned with patient goals. Identifies and closes gaps in care. Addresses patients’ and families’ physical and psychosocial needs with interdisciplinary team input. Educates patients and families about medication usage, side effects, illness progression, diet, nutrition, medication adherence, and crisis prevention. Coordinates care with other clinical team members and external providers to meet patient and family needs timely and in compliance with regulations. Maintains patient medical records and documentation per state regulations and company policy. Participates in continuing education required by state and certifying bodies. Prescribes medication as authorized by state prescribing authority. Minimum Requirements MS in Nursing. Active, national NP certification. Valid, current, active, and unrestricted Family or Adult Nurse Practitioner license in Arizona, Kansas, Iowa, Indiana, Ohio, and Florida. Valid, current, active, RN Compact license. At least 2 years of experience managing complex care cases. Experience working with Electronic Medical Records (EMR). Preferred Skills, Capabilities and Experiences DEA registration or eligibility preferred. Active Medicaid number in Arizona, Kansas, Iowa, Indiana, Ohio, and Florida highly preferred. Experience managing complex care cases for developmental disabilities and chronically ill patients. Experience working in a hospice environment.
