Join our growing team! We are looking for a dedicated and experienced LPN for our Home Health Intake Department. The LPN is responsible for coordination of home care for new and current patients. This position is full-time and required to work in the corporate office.

• Reviews incoming referrals to ensure all adequate documentation and orders have been received and evaluates for appropriateness of home care.
• Assists the physicians, field staff and therapy department in coordination of patient care, including but not limited to labs, wound care orders, DME, supplies, and additional discipline needs.
• Communicate with other departments regarding status of referrals.
• Assists field staff with reporting patient problems/concerns to the physician
• Performs other duties as requested/assigned.
QUALIFICATIONS:
• Required graduate of an accredited school of nursing.
• Required current license to practice as a licensed practical nurse in Louisiana
• Required License must be unencumbered.
• Minimum of one year nursing experience.
SKILLS:
• Demonstrates an ability to work with other health care clinicians.
• Demonstrates good time management.
• Ability to read and communicate effectively in English
• Excellent interpersonal, telephone, documentation, and communication skills.
• Excellent computer skills (i.e., Word, Excel, Outlook, Power Point, etc.)
