Description

• Conduct assessments of patients in both inpatient and outpatient settings while crafting personalized treatment strategies
• Deliver interventions targeted on mobility, strength, balance, and functional performance
• Work collaboratively with nurses, physicians, case managers, and fellow rehabilitation professionals to maximize patient outcomes
• Provide education to patients and their families regarding therapeutic objectives, the discharge process, and at-home exercises
• Maintain accurate records of evaluations, treatments, and patient progress in line with facility and regulatory standards
• Engage in interdisciplinary conferences and care team rounds to ensure seamless care delivery

Education

• Doctorate in Physical Therapy (DPT) or equivalent from a recognized institution

Certifications

• Current state licensure as a PT / Physical Therapist
• CPR certification preferred
• Specialty certifications (e.g., neurological, geriatric) are advantageous but not mandatory

Skills

• Preferred prior experience in a clinical setting
• Capable of treating patients with complex medical and mobility challenges
• Excellent interpersonal and communication skills
• Familiarity with digital charting and clinical documentation tools
