We are hiring a dedicated and experienced Nurse Practitioner to join a PACE (Program of All-Inclusive Care for the Elderly) team. This role focuses on delivering comprehensive care to an elderly population through a collaborative, interdisciplinary approach. Key Responsibilities:

We are hiring a dedicated and experienced Nurse Practitioner to join a PACE (Program of All-Inclusive Care for the Elderly) team. This role focuses on delivering comprehensive care to an elderly population through a collaborative, interdisciplinary approach. Key Responsibilities:
- Provide primary and preventive care, including diagnosing and managing chronic and acute conditions.
- Collaborate with interdisciplinary teams to support holistic care plans.
- Refer patients to behavioral health professionals as appropriate.
- Supervise nursing staff and support staff in clinical settings.
- Ensure timely and thorough documentation within the EMR.
- Participate in quality improvement efforts and the development of care protocols.
- Provide inpatient and on-call coverage as assigned.
- Minimum: Licensed Nurse Practitioner or Licensed Independent Practitioner
- Preferred: Master of Science in Nursing (MSN), National Certification as a Family Nurse Practitioner
- Required: Experience in a community clinic and/or managed care setting
- Preferred: Experience working with elderly patients
- Required: RN license and current CPR certification
- Preferred: MSN
- Supervisory experience with nurses
- Bilingual (English/Spanish) preferred
- Strong oral and written communication skills
- Proficient in wound care; ACLS certification preferred
