Job Description

Key Responsibilities:

• Implement physician-approved care plans
• Carry out physical therapy treatment plans established by the supervising Physical Therapist and physician, delivering skilled interventions in the patient's home.
• Deliver direct therapeutic treatment
• Administer therapeutic exercises, manual therapy, massage, gait and balance training, and physical modalities (heat, cold, ultrasound, electrical stimulation) to relieve pain, restore function, and improve mobility.
• Provide assistive device training
• Instruct patients on the safe use of walkers, crutches, canes, wheelchairs, braces, and prosthetics to support independent functioning at home.
• Educate patients and families
• Teach home exercise programs, proper body mechanics, and medical equipment safety to patients and caregivers to promote safe, confident recovery between visits.
• Monitor and document patient progress
• Observe patients before, during, and after each session; record clinical findings; and submit timely progress notes to inform the supervising PT and care team.
• Communicate changes in patient condition
• Report any changes in a patient's status or response to treatment to the Physical Therapist promptly, recommending plan-of-care modifications as needed.
• Coordinate interdisciplinary care
• Participate in care conferences, collaborate with agency personnel (nursing, OT, social work), and keep the team informed of patient progress to ensure seamless, holistic care.

Qualifications

Qualifications:

• Be able to implement care plans and communicate any findings or changes in regards to patients back to the Physical Therapist and physician
• Ability to educate patients and their families in basic home safety and medical equipment safety

Required Certifications and Licensures:

• Associates degree from an accredited school of Physical Therapy
• Have a valid license, registration, or permit to practice in state of agency operation
• Must possess and maintain valid CPR certification while employed in a clinical role
• Must be a licensed driver who can travel to all business locations
