Case Manager

Position Summary The Case Manager engages consumers in the discharge planning process and supports their transition to the community, primarily through direct in‑home contact and linkage to community resources. The role includes assessments within correctional facilities, attending court hearings, and advocacy. The manager remains on call for client crisis needs and works closely with probation, jail, and other community agencies to secure referrals and facilitate pre‑ and post‑discharge planning. Responsibilities Conduct assessments, develop service plans, and link clients to resources. Provide weekly face‑to‑face contact with clients, using treatment plan goals for session preparation. Arrange transport, facilitate warm handoffs, and follow up on resource needs. Maintain accurate documentation in line with state contract and program manual. Monitor treatment planning, ongoing support, and discharge planning. Participate in on‑site presence at jail and communicate consistently with probation and community agencies. Attend program, agency, and supervision meetings. Complete all required case‑management trainings and documentation within required timeframes. Adhere to flexible schedule, program policies, and budgetary guidelines. Work collaboratively with coworkers and agency staff to problem‑solve and support client needs. Qualifications Bachelor of Arts in a behavioral health field from an accredited institution and at least one year of post‑degree experience – OR – Associate’s degree in a direct care field and at least two years of post‑degree experience – OR – Minimum high school diploma with lived experience of recovery and completion of a nationally or NJ‑approved peer certification within one year of hire. Valid driver’s license with an acceptable driving record. Strong independent thinking, judgment, analytical, and goal‑setting abilities. Excellent written and verbal communication skills. Benefits and Compensation Base Rate: $21.85 per hour. Sign‑on Bonus: $750. Schedule: Day.
