In our medical clinics, we provide comprehensive primary care for senior adults with Medicare Advantage plans, focused on delivering improved quality, better patient experience and lower total cost of care./ppReporting to the Center Manager, the Registered Nurse (RN) has daily responsibility in direct patient care and education and fostering collaborative relationships with providers and other clinical operations staff. This position is ideal for an experienced nurse who enjoys performing daily patient care, has a passion for working with the senior community, and desires a day shift/Monday-Friday schedule./ph3Duties/Responsibilities:/h3ulliResponsible for adhering to the clinical team workflows./liliPerforms daily staff nursing activities; day-to-day patient activities, including phone and in-person patient triage and patient education./liliCollaborates with the Providers and Center Manager to identify opportunities to improve clinical performance of back-office personnel and functions. Develops, implements, and monitors the effectiveness of performance improvement activities./liliPerforms regular monitoring activities to ensure compliance with national and organizational standards of care. Clinically prepares the market for internal and external quality audits and helps lead the center response to any clinical deficiencies found./liliMaintains current knowledge of state regulations for nursing, medicine, and pharmacy as it relates to scope of practice for back-office personnel./liliIdentifies and provides appropriate resources to educate patients, their families and staff members on clinical conditions and processes./liliOversees clinical quality and patient safety through training and education of the back-office personnel and administrative staff. Communicates effectively in all areas of practice, including how to motivate, and engage colleagues around a common goal./liliEnsures that clinical processes, policies, and procedures are adhered to in the center./li/ulh3Required Skills/Abilities:/h3ulliStrong knowledge of healthcare functions, including clinical functions, practice safety processes, reimbursement practices, and quality improvement initiatives./liliTrack record of developing and implementing processes for providing excellent customer service./liliSuperior clinical/patient care skills./liliThorough, in-depth knowledge of clinical procedures, legal rules and regulations and best practices in healthcare./liliSkilled in observation, training and evaluating clinical/patient care competencies./liliAbility to effectively leverage business and organizational knowledge within and across functional areas./liliMust possess a high degree of emotional intelligence and integrity, driven and focused work ethic./liliContinuous desire to learn and embrace new methods; ability to adapt and be resilient./liliSelf-starter with the ability to think creatively and work effectively./liliAbility to build a relationship and work effectively with various seniorities and diverse populations including staff, providers, members, and family members./liliExcellent critical reasoning, decision-making, and problem-solving skills to analyze situations, determine risks, and find solutions to prevent future issues and resolve recurring defects./liliProven skills in budgeting and fiscal management preferred./li/ulh3Education and Experience:/h3ulliAA/AS degree in Nursing required; BA/BS degree in Nursing (BSN) or Healthcare Administration preferred./liliA valid, active Registered Nurse (RN) license in state(s) of employment required./liliA minimum of 5 years, current direct nursing/medical work experience required./liliWork in a community-based clinic, public health department or specialization with the senior population preferred./liliBLS certification for healthcare providers required./liliA problem-solving orientation and eagerness to identify process gaps and implement practical solutions in a matrixed organization./liliProficient PC skills./liliFluency in Spanish or other languages spoken by people in the communities we serve is helpful./li/ulh3Embodies and serves as a role model of ArchWell Healths Values:/h3ulliBe compassionate./liliStrive for excellence./liliEarn trust./liliShow respect./liliRemain resilient./liliDo the right thing and do it with passion./li/ulpArchWell Health is an equal opportunity employer.
