Primary Care Physician, Pod Leader

Location: San Jose, California. Relocation Assistance Available

Base Salary - $308,000 to $363,000

Bonus Eligible Ideal Candidate

• 2-5 years of experience in primary care focused on geriatric care.
• Worked in an Interdisciplinary Team structure (IDT)
• Led Nurse Practitioners or Physicians Assistants

Details: Seeking a Physician Pod Leader to provide community-based care to our California's most vulnerable seniors. As part of their mission-driven interdisciplinary team, the Primary Care Physicians can build deeper, long-term relationships with fewer patients than in a traditional healthcare model, allowing you to see the impact of your care every day. Essential job duties include:

• Perform comprehensive in-person assessments, leading and partnering with the interdisciplinary team (IDT) to develop a personalized care plan for each participant
• Coordinate discharge for acute, short-term, and long-term placement
• No hospital rounds focus on comprehensive care, seeing an average of 8 patients a day

Job requirements include:

• Primary Care Physicians in Internal Medicine, Family Medicine, Emergency Medicine or Geriatrics.
• Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
• Master's or doctorate Degree
• Board Certification preferred
• Active state physician license without restrictions
• State CDS certification and DEA registration
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
• Must be eligible for enrollment to provide services to Medicare and Medi-Cal participants.

Salary and benefits:
