Description

• Assess patients in either an inpatient or outpatient context and create tailored treatment plans
• Implement physical therapy interventions concentrating on mobility, strength, balance, and overall function
• Collaborate seamlessly with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
• Provide education to patients and their families regarding therapeutic objectives, discharge plans, and home exercise routines
• Maintain accurate documentation of evaluations, treatments, and progress per facility and regulatory standards
• Engage in care team discussions and interdisciplinary meetings to ensure synchronized treatment

Education

• Doctorate in Physical Therapy (DPT) or an equivalent from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (e.g., focused on neurology or geriatrics) are advantageous but not essential

Skills

• Prior experience is highly preferred
• Ability to manage patients with intricate medical and mobility concerns
• Exceptional communication and teamwork capabilities
• Adeptness with digital charting and clinical documentation systems
