Case Manager II — Supports Independent Living & Advocacy

Seven Hills Foundation invites a dedicated Case Manager II to Rhode Island to oversee Shared Living/Adult Family Care coordination. You will ensure high-quality, person-centered services promoting independence, health, and safety while meeting regulatory requirements. You’ll develop plans, advocate for rights, and coordinate a team of providers to support individuals and families.
