Help make behavioral health care accessible when clients need it most—and support the therapists who make that possible. What happens when someone is discharged from the hospital after a serious medical event, is recovering following surgery, or is experiencing significant or acute medical challenges, but their mental health needs don't stop and getting to a traditional therapy appointment simply isn't realistic? That's where Integrated Care comes in. Center for Life Management is seeking an Integrated Care Clinical Supervisor to provide clinical leadership and supervision to a team serving individuals living with serious and persistent mental health conditions and co-occurring chronic physical health conditions that significantly affect their daily functioning and ability to access care. This is an opportunity for an experienced therapist who wants to combine clinical leadership, staff development, and direct client care. You'll support therapists in delivering high-quality, person-centered treatment while continuing to work directly with clients and staying connected to clinical practice.

Integrated Care is built around meeting clients where they are. That may mean seeing a client in our office. It may mean providing therapy through telehealth. And when significant medical or functional challenges prevent someone from accessing care in those settings, it may mean meeting them in their home or another community setting. We help clients build greater independence, reconnect with their communities, and access treatment in the least restrictive setting appropriate for them, while recognizing that clients living with complex or chronic medical conditions sometimes need us to bring care to them.

As an Integrated Care Clinical Supervisor, you'll help ensure both clients and therapists have the support they need to be successful. You'll:

• Provide regularly scheduled clinical supervision and ongoing supervisory support to designated clinical staff and clinical interns. 
• Orient, train, mentor, and support new and existing team members in developing their clinical skills and providing high-quality Integrated Care services.
• Provide ongoing coaching and feedback to staff and complete formal performance evaluations. 
• Supports effective service delivery, client outcomes, productivity expectations, and overall program performance. 
• Foster a positive and collaborative team culture that promotes professional growth, teamwork, accountability, and high-quality client care. 
• Provide individual and group therapy to adults with serious and persistent mental health conditions and co-occurring chronic physical health conditions. 
• Conduct behavioral health assessments and develop individualized treatment plans that recognize the relationship between mental health, chronic physical health conditions, and overall functioning. 
• Ensure therapists provide high-quality assessment, treatment planning, therapy, crisis intervention, and other clinical services consistent with client needs and program expectations. 
• Support clients whose acute or chronic medical circumstances create barriers to participating in traditional office-based behavioral health treatment. 
• Provide services flexibly in the office, clients' homes and community settings, and through telehealth based on clinical needs and treatment goals. 
• Provide crisis assessment and intervention when clients experience acute behavioral health needs. 
• Collaborate with primary care physicians, specialists, behavioral health providers, families, guardians, natural supports, and other members of the treatment team to promote continuity and coordination of care. 
• Ensure timely, accurate clinical documentation and adherence to agency, state, and federal requirements. 

Integrated Care isn't a one-size-fits-all program, and neither is the work. Services are provided across the office, community, clients' homes, and telehealth depending on the needs of the individuals we serve. That flexibility is an important part of the position and an important part of how we remove barriers to behavioral health care. We're looking for an experienced therapist who enjoys helping other therapists grow while still valuing the opportunity to provide direct client care. You may be a great fit if you:

• Hold a master's degree in social work, clinical mental health counseling, psychology, or a related mental health field and hold appropriate clinical licensure. 
• Have at least four years of full-time clinical experience following completion of your master's degree, including at least two years of full-time clinical experience following licensure. 
• Have experience providing clinical supervision, leadership, mentoring, or staff development, or are ready to bring strong clinical leadership skills into a formal supervisory role. 
• Understand chronic and co-occurring physical health conditions, social determinants of health, and the ways these factors can affect an individual's mental health, functioning, and access to care. 
• Have experience working with medically complex individuals and/or individuals with co-occurring disorders. 
• Can exercise sound clinical judgment in routine, urgent, and emergent situations and help other therapists strengthen their own clinical decision-making. 
• Enjoy developing therapists through supervision, coaching, feedback, and ongoing learning. 
• Believe people should be able to access quality behavioral health treatment even when health, transportation, mobility, or other circumstances make traditional services difficult to access. 
• Are comfortable providing and overseeing services across a variety of settings, including the office, community, clients' homes, and telehealth.
• Can balance the needs of clients, staff, and the program while remaining organized, responsive, and collaborative. 
• Want to be part of a multidisciplinary approach that recognizes the connection between mental health, physical health, social determinants, and overall functioning. 

This is a role for a therapist who wants to lead without stepping away from clinical practice. You'll have the opportunity to develop and support other therapists, help shape the quality of services delivered by the Integrated Care team, and continue working directly with individuals whose behavioral and physical health needs intersect. Physical Demands and Working Conditions

A majority of the work will be performed in various settings including the office, community, client homes as necessary, and/or remotely via telehealth. Manual dexterity is needed to use computers. The ability to sit for extended periods of time is required. Must be able to flex schedule to accommodate needs of clients and staff. The ability to sit for extended periods of time is required. Requirements:

Qualifications and Skills

• Master’s degree with licensure in mental health field.
• Four years of full-time clinical experience post Master’s degree.
• Two years of full-time clinical experience post licensure.
• Experience working with co-occurring disorders preferred.
• Knowledge of chronic comorbid physical health conditions, social determinants of health and experience with medically complex individuals.
• Ability to exercise sound clinical judgment in routine, urgent, and emergent clinical situations.
• Proficient computer skills and knowledge of all Microsoft office applications (Word, Excel, Publisher, Power Point, Outlook, TEAMS, etc.).
• Ability to document effectively and efficiently in course of clinical work.
• Ability to navigate an electronic health record.
• Advanced documentation skills and familiarity with data reports/spreadsheets.
• Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision.
• Ability to acquire and effectively practice new clinical skills as needed according to evolving agency needs/services.
• Ability to demonstrate strong leadership skills that guide the implementation and sustainability of program services and foster workforce development.
• Demonstrate effective communication skills, organizational skills, and ability to multi-task.
