Job Summary and Responsibilities As our Family Medicine Advanced Practice Provider you will deliver exceptional primary care within our Family Medicine Department in Phoenix, Arizona. Clinic-Based Family Medicine (approx. 50% of time) Provide comprehensive primary care to patients of all ages (infants to geriatrics), including preventative care, acute and chronic disease management, and health education Conduct thorough physical examinations, order and interpret diagnostic tests, and develop individualized treatment plans Prescribe medications and treatments in accordance with state regulations and practice protocols Perform minor in‑office procedures (e.g., Derm/MSK procedures and Nexplanon]) Maintain accurate and timely electronic health records (EHR) Collaborate effectively with physicians, nurses, MAs, and other healthcare professionals Participate in team meetings, quality improvement initiatives, and continuing education Virtualist Program (approx. 50% of time) Must live in the Phoenix, AZ area Provide virtual primary care consultations via secure telehealth platforms (video, phone, secure messaging) Assess, diagnose, and treat a wide range of acute and chronic conditions amenable to virtual care Order necessary labs, imaging, and prescriptions remotely Provide appropriate referrals to specialists or in‑person care when clinically indicated Educate patients on their health conditions, treatment plans, and self‑management strategies Adhere to all telehealth best practices, privacy regulations (HIPAA), and state licensing requirements for virtual care Utilize virtual care tools and technology proficiently Benefits: full benefits, CME allowance, malpractice with tail coverage, relocation assistance for eligible candidates. Job Requirements Required Current and unrestricted licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in the State of Arizona Current national board certification (ANCC, AANP, NCCPA) BLS certification Minimum of 3 years of experience as an Advanced Practice Provider in Family Medicine or Primary Care Preferred Proficiency with Electronic Health Records (EHR) systems; experience with Cerner is a plus Tech‑savvy with the ability to quickly learn and adapt to new technologies #J-18808-Ljbffr Dignity Health Medical Group
