Concierge Home Care is hiring a Home Health Physical Therapist (PT) . If you’re looking for more control over your schedule, meaningful patient relationships, and the ability to increase your income based on productivity—this is the role for you.
Coverage Area: Vero Beach / Indian River County
( Territories designed to minimize drive time with clustered scheduling )
Compensation & Earning Potential
- Pay-per-visit model: $70+ per visit + Medicare differential pay

Your Role as a Home Health Physical Therapist
- Evaluate patients and develop individualized treatment plans
- Provide therapeutic exercises and mobility training
- Educate patients and caregivers on safety and recovery
- Collaborate with physicians and interdisciplinary teams
- Supervise Physical Therapist Assistants (PTAs)
- Complete documentation using streamlined technology

- Active, unencumbered Florida Physical Therapist license
