Licensed Practical Nurse

Conducts independent home visits for veterans with complex medical and psychosocial needs, delivering direct nursing care and providing education to patients and caregivers in alignment with the individualized care plan established by the RN case manager.

Collaborates with the HBPC interdisciplinary team including the physician preceptor, registered nurse, licensed practical nurse, occupational therapist/physical therapist, social work, dietitian, speech therapist, psychologist, pharmacist, and advanced practice provider.

Collaborates with services internal and external to the VA to manage the care needs of the patients. Provides patient and family health education with a focus on self-management, prevention, health promotion, and wellness based on the patient's individual goals.

Supports additional teams by completing home visits as needed based on patient care demands.

Demonstrates strong prioritization skills in determining the order of patient visits according to acuity and need.

Utilizes HBPC policies and procedures to provide autonomous, high-quality care in the home setting.

Completes the basic foot care course within the first year as a requirement of this LPN role.

This position includes performing traditional HBPC home visits, wound care home visits, and specialized podiatry LPN home visits.

Work schedule: Monday - Friday: 8am - 4:30pm
