At Home Care Partners is seeking a compassionate and experienced Home Care Field Registered Nurse (RN) to provide clinical care and support to patients throughout Schoharie County, New York .

This full-time opportunity is a great fit for an RN who enjoys working independently, connecting directly with patients and families, and using their clinical expertise in home and community-based settings.

As a Field RN , you will travel throughout Schoharie County to conduct nursing assessments, oversee individualized Plans of Care, supervise home health aides, and help ensure patients receive safe, personalized, and high-quality care.

What You'll Do

• Travel to patient homes and community-based settings throughout Schoharie County

• Conduct comprehensive in-home nursing assessments

• Perform head-to-toe evaluations, including medication and diagnosis reviews

• Develop, review, and update individualized Plans of Care (POC)

• Complete required home health aide supervisions

• Provide clinical guidance and support to home health aides and caregivers

• Orient aides to each patient's Plan of Care and specific care needs

• Monitor patient conditions and report significant changes or concerns

• Communicate with patients, families, caregivers, aides, and office staff

• Complete accurate and timely clinical documentation

• Help ensure services are delivered safely and according to each patient's individual needs

• Maintain compliance with applicable healthcare regulations and company policies
  

What We're Looking For

• Current and active New York State Registered Nurse (RN) license

• Ability and willingness to travel throughout Schoharie County and assigned service areas

• Medical Professional Liability insurance

• Strong clinical assessment and critical-thinking skills

• Excellent written and verbal communication skills

• Strong organizational and documentation abilities

• Comfortable using Gmail and Microsoft Word/Excel

• Ability to work independently and manage a field-based schedule

• Professional, dependable, organized, and compassionate

• Must meet all company hiring and onboarding requirements
  

Preferred Experience

• Previous home care or home health nursing experience

• Community health or outpatient nursing experience

• Conducting in-home nursing assessments

• Completing aide supervisions

• Developing, reviewing, or updating Plans of Care

• Familiarity with Medicaid, Medicare, or MLTC plans

• Bilingual Spanish/English is a plus
