Pharmacist - San Diego, CA

A California-licensed pharmacist is needed to join a remote Medication Therapy Management (MTM) pilot program, supporting clinical pharmacy initiatives and patient counseling. This part-time contract opportunity offers flexible scheduling of approximately 10-20 hours per week, with a guaranteed minimum of 8 paid hours weekly, allowing you to work from anywhere within California. This role is ideal for pharmacists with MTM experience who are enrolled as an ORP provider in California Medicaid (Medi-Cal).


Responsibilities include:

• Conducting comprehensive MTM consultations
• Providing patient counseling and medication education
• Reviewing medication profiles to identify optimization opportunities
• Documenting clinical interventions and patient encounters with accuracy
• Collaborating with program stakeholders to enhance patient outcomes
• Ensuring compliance with state and federal pharmacy regulations

Desired qualifications for this position are:

• Active, unrestricted California Pharmacist license
• Residency within California
• Previous experience in Medication Therapy Management
• Current enrollment in California Medicaid (Medi-Cal) as an ORP (Ordering, Referring, and Prescribing) Provider
• Strong communication and patient counseling abilities
• Capability to work independently and effectively in a remote setting with flexible hours

Schedule/Hours:
