Social Worker Assistant - SW I (PCN 137) Full Time Professional Van Nuys, CA, US Salary Range: $29.25 To $31.00 Hourly The Social Worker I (Social Work Assistant) for the Central Access department will provide responsive services to individuals seeking social service or other community-based assistance or information. She/he will be responsible for telephone response including information and referrals to clients. The SWI will assist Social Workers in the provision of short-term case management tasks. Tasks will include assistance with client enrollment in eligible public benefits, applications for applicable grants assistance, research on available services or information clients may need to access concrete, social, psychological, medical, and economic resources available in the community. Field based services are provided to clients at one of two SOVA Food Pantry sites, or at community-based pop‑up service sites. Qualifications BA degree in social work or related health or human service field. Prior experience with client engagement, customer service, or social service interventions preferred. Demonstrated strong interpersonal and customer service skills, with the ability to work effectively and respectfully with a multicultural client population, engage in a culturally sensitive and nonjudgmental manner that supports positive outcomes, and work collaboratively as part of a team while maintaining appropriate professional boundaries. Excellent communication skills, both verbal and written, and ability to utilize telehealth platforms. Current unrestricted CA Driver’s License and proof of auto insurance. Must pass a criminal background check (Live Scan fingerprinting) prior to beginning employment. Responsibilities / Essential Functions Respond to intake calls in a timely and professional manner to assess needs and provide information and referrals. Provide connections to internal JFSLA programs and community referrals as indicated. Provide direct front‑facing client engagement and customer service at JFSLA SOVA sites and community‑based pop‑up locations. Provide short‑term targeted case management services, including ongoing follow‑up and development of individualized case management plans and goals in collaboration with clients. Support clients in identifying available government and community resources and assist, as needed, with advocacy and navigation. Work collaboratively with SOVA staff on outreach efforts, client registration, and client engagement to help connect community members to additional targeted case management services. Manage multiple tasks and prioritize effectively. Provide timely documentation in accordance with funding requirements and electronic health record standards. Participate in weekly individual supervisory meetings. Other related duties as assigned.
