Schedule/Hours:

• Two assignment structure options: Guaranteed hours or flexible/non-guaranteed hours
• If guaranteed: Minimum productivity requirement of 27 points per week; must be willing to travel or work weekends to fulfill caseload, except during holidays or approved time off
• Flexible assignment available with optional weekend coverage, ability to secure full caseload by traveling beyond assigned territory

Qualifications:

• Current California Occupational Therapist license
• BLS certification
• Valid driver's license, auto insurance, auto registration
• Annual TB test, MMR, Varicella, HepB, TDap (within 10 years), flu vaccine
• Ability to pass a drug screen and physical
• Home health experience a plus, but not required
• Dedication to caring for diverse patient needs, including willingness to see Covid+ and Covid PUI patients

Core Responsibilities:

• Provide evaluation, treatment, and follow-up for home health patients
• Complete documentation and communication (including with patients' physicians)
• Adapt care plans to each patient's goals and living situation, maintaining compliance with all clinical protocols and productivity standards
• Flexibly adjust to weekend or expanded area coverage to maximize caseload and meet community needs
