Case Associate

Role Overview The Case Associate provides administrative and clerical support for care coordination, including managing client intake and referrals, maintaining files and billing data, responding to inquiries, and collaborating with internal and external stakeholders to ensure timely and accurate service delivery. Job Responsibilities Provide administrative support to the care coordination process; prepare and maintain program data and billing; monitor and respond to client inquiries, concerns and complaints. Responsible for intake, processing of client referrals, and preparation of client files. Ensure timely follow‑up to messages/inquiries made to the care coordination team. Serve in a clerical capacity to review and monitor files and related documentation. Complete assigned billing tasks including data entry and maintenance. Provide pre and post assessment activities. Work with external partners to resolve billing matters. Attend all trainings as required. Provide support to clients, care coordinator, supervisors and management staff. Work collaboratively with internal stakeholders and team members as needed. Engage in ongoing learning, training, and skill development, seeking and applying feedback to continuously improve performance and contribute to team and organizational success. Physical Requirements Kneel and move from sitting, bending, kneeling, or standing multiple times a day Climb up and down up to 3 flights of stairs at a time Lift, push, and pull up to 25 lbs Other Requirements Background check Education and Experience Requirements Minimum H.S. Diploma or General Education Degree (GED) Strong communication, problem solving skills and critical thinking skills needed Previous data entry skills are required.
