Description

• Conduct evaluations for patients in either inpatient or outpatient settings and develop tailored treatment plans
• Provide physical therapy interventions aimed at enhancing mobility, strength, balance, and functionality
• Collaborate with nurses, doctors, case managers, and other rehabilitation specialists to maximize patient outcomes
• Inform patients and their families about therapeutic objectives, discharge strategies, and home exercise regimens
• Maintain documentation of evaluations, treatments, and progress in accordance with facility and regulatory standards
• Engage in team care rounds and interdisciplinary conferences to ensure coordinated treatment

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., geriatric, neurological) are advantageous but not mandatory

Skills

• Prior experience is highly desirable
• Demonstrated ability to manage patients with intricate medical and mobility challenges

• Proficient with digital charting and clinical documentation systems
