Job Summary:
Experienced Home Healthcare Registered Nurse (RN) needed for a facility in Portland, Oregon. This position involves providing patient-centered care in a home health setting, emphasizing assessment, wound care, medication management, and patient education. The role requires maintaining clinical excellence while working independently within a community-based environment.

Job Details:

• Schedule: 8-hour days, minimum of 40 hours per week, start date 08/10/2026
• Required Qualifications: Oregon State licensure, BLS certification, at least 1 year of skilled home health experience, reliable transportation, and Oasis system experience
• Preferred Qualifications: Experience with adult and geriatric patient populations, chronic disease management, and familiarity with community hospital settings
• Float: No
• Additional Information: The traveler will visit up to 6 patients per day, with an expected productivity of 30 points. Occasional travel radius of approximately 60 miles per day. COVID-19 vaccination series (primary and booster) required. The role involves charting via EPIC Remote Client and Rover mobile app. The facility emphasizes minimal drive time through efficient client grouping. The traveler will need their own transportation to meet patient care responsibilities efficiently.
